Jack A. Stevens, 61, of Carmi, passed away Saturday, March 24, 2012 at 5:29 p.m. at his residence. He was born August 18, 1950 in Evansville, IN, the son of Martin and Lenore (Martin) Stevens. Jack married Rita Sue Prater July 26, 1999 and she survives. He was a U.S. Army veteran serving with in the 173rd Airborne during Vietnam. Jack was an equipment operator at the Willow Lake Mine in Equality.
Jack is survived by his wife, Sue; a daughter, Natalie Stevens of Davenport, IA, two step daughters, LaShonda Johnson and Carmen Elam both of Maysville, KY;and a brother and sister-in-law, Joe and Terri Stevens of Crossett, AR; a sister-in-law, Loretta Stevens of St. Louis, MO; a step granddaughter, Leeanna Holler; three nieces; and three nephews. He was preceded in death by his parents and a brother, Jeff Stevens.
Funeral services for Jack A. Stevens, 61, of Carmi, will be Wednesday, March 28, 2012 at 1:00 p.m. at Campbell Funeral Home in Carmi. Burial will be in Union Ridge Cemetery in Norris City with military rites by Carmi VFW and American Legion. Friends may call on Wednesday from 12:00 p.m. until time of service at the funeral home.
Memorial gifts may be made in Jack's name to Pets At Risk and will be accepted at Campbell Funeral Home.
